Magnesia phosphorica is the great antispasmodic of the materia medica — the remedy of cramping, shooting, lightning-like pains that shift about, come and go abruptly, and are relieved by heat, hard pressure and bending double. Boericke opens his entry with exactly that claim: "The great anti-spasmodic remedy. Cramping of muscles with radiating pains. Neuralgic pains relieved by warmth." Nash calls it "the prince of the Magnesias" and places it in the first rank of pain remedies. This guide assembles its keynotes from the public-domain authorities, sets out the modalities that carry the prescription, and works through the differential that decides most Mag phos cases at the bedside — Colocynthis — as education for practitioners and serious students, not as self-treatment advice for the public.

The Substance and Its Sphere

Magnesia phosphorica is the phosphate of magnesia — MgHPO₄·7H₂O, prepared by trituration (Clarke). It entered homeopathy through Schüssler as one of the twelve tissue remedies, and Hering's Guiding Symptoms still describes it as awaiting a full proving. That gap was closed by H. C. Allen, whose proving in the potencies Clarke cites as the foundation of the modern picture.

The sphere is the nerve and the muscle. Schüssler's own statement, quoted by Clarke, is that the phosphate of magnesia is contained in the blood corpuscles, muscles, brain, spinal marrow, nerves and teeth, and that disturbance of it results in pains, cramps and paralysis. Everything the remedy does follows from those three words. Kent puts it in his own idiom: "Magnesia phos. is best known for its spasmodic conditions and neuralgias" — a pain localises in a nerve, worsens paroxysm by paroxysm, and becomes so violent that the patient becomes frantic.

Mag phos is not a polycrest with a deep constitutional picture. It is a great acute remedy of pain and spasm, read from the character and modality of the pain, not from the patient's inner life.

The Character of the Pain

Nash's list is worth learning verbatim. The pains are "sharp, cutting, piercing, stabbing, knife-like, shooting, stitching, lightning-like in coming and going (Belladonna), intermittent, the paroxysms becoming almost intolerable, often rapidly changing place and cramping." He adds: "This last is in my opinion most characteristic, and is oftenest found in stomach, abdomen and pelvis." Allen sharpens two details — the paroxysm becomes almost unbearable, driving the patient to frenzy, and the pain arrives with a constricting sensation, as though a band were drawn tight, which Schüssler had already noticed: shooting like lightning, or boring, "often combined with or alternating with a sensation of constriction; at times wandering" (Clarke).

Three qualities define the Mag phos pain:

  • It cramps. Whatever else it does, it grips and constricts. Allen's image for the gastric spasm is exact — "as if a hand was drawn tightly around body."
  • It is lightning-like in coming and going. Sudden onset, sudden cessation, no slow build — the quality Nash and Allen both cross-reference to Belladonna.
  • It shifts. Allen's provers had pains that "rapidly change place" (Clarke) — a toothache moving from tooth to tooth, a neuralgia migrating around the face.

One quality is absent: burning — and Nash built a working diagnostic on it, below.

Modalities: the Whole Remedy in One Line

Boericke's summary is the shortest useful statement of the remedy: worse right side, cold, touch, night; better warmth, bending double, pressure, friction.

Allen expands the aggravations into their causes — cold air, a draught of cold wind, cold bathing or washing, motion, touch — against the ameliorations that Clarke reports Allen printed in capitals: bending double, heat, warmth, pressure. Clarke adds two aggravations easily missed: lying on the back stretched out, and aggravation while eating.

Two points deserve care.

The touch paradox. Mag phos is worse from touch and better from pressure. These are not contradictory: Schüssler's line is better for pressure, worse from light touch. The facial neuralgia is exquisitely sensitive to a light contact — yet the same patient presses the whole palm hard into the abdomen and sighs with relief. When the case reports both, that is confirmation rather than confusion.

The right side. Allen states it as a general: affections of the right side of the body — head, ear, face, chest, ovary, sciatic nerve — and Boericke, Clarke and Kent repeat it region by region, from supraorbital pain worse right to dysmenorrhoeic pain worse right.

Constitution, Causation and Mental State

Allen's constitutional note is that Mag phos is best adapted to thin, emaciated persons of a highly nervous organisation, of dark complexion — which Clarke immediately qualifies with Allen's own caveat that it "acts promptly in stout, fleshy persons when well indicated." Hering's authorities gave it to young and very strong persons and to teething children. The constitutional filter is weak here; the modality is the prescription.

The state that does recur is exhaustion. Boericke: "Especially suited to tired, languid, exhausted subjects. Indisposition for mental exertion." Clarke quotes the proving directly — "Languid, tired, exhausted; unable to sit up" — and notes that attacks often come with great prostration, and that the affections are frequently periodic.

Clarke's causations are a checklist worth keeping: dentition, cold winds, cold bathing, standing in cold water, working with cold clay, study, catheterism. The last two are almost pathognomonic.

The mind is the mind of someone in pain. Boericke: "Laments all the time about the pain. Inability to think clearly." Clarke adds sobbing and lamenting about the pain in the affected parts, often with hiccough; depression and anxiety; drowsiness on every attempt to study. There is no elaborate delusion here to prescribe on, which is precisely why the modality discipline matters.

Regional Pictures

Head and face

The Mag phos headache is neuralgic or rheumatic and, in Clarke's phrase, always better for the "external application of warmth." Allen locates it: begins in the occiput and extends over the head, face red and flushed, from mental emotion, exertion or hard study, better for pressure and external heat. Clarke's schoolroom version — headache "beginning in, or worst in occiput, and constant whilst attending school" — is one of the remedy's most reliable clues in students, and Kent adds the posture: the patient wants "the head bandaged with a tight-fitting cloth, a warm room."

Facial neuralgia is one of the remedy's strongest fields. Kent: neuralgia of the face, worse on the right, ameliorated by heat and pressure, aggravated by cold; tic douloureux; "It has cured more face-aches than other pains." Clarke's provers supply two separate details: pains radiating all over the right side of the face from the infra-orbital foramen, worse from touch, from opening the mouth to eat or drink and from cold air; and a neuralgia of the right upper jaw and teeth that begins with greatest fierceness at 2 p.m. and lasts until he gets warm in bed, the pains sharp and lightning-like. Boericke's ear note belongs to the same complex: severe neuralgic pain worse behind the right ear, worse from cold air and from washing the face and neck with cold water.

Teeth and teething

Boericke gives the keynote in a single line: "Toothache; better by heat and hot liquids." Allen adds that it is worse at night, rapidly shifting, and worse from cold things; Clarke's proving produced severe pain in decayed or filled teeth in seven provers. For teething, both Boericke and Allen record spasms during dentition without febrile symptoms — and Clarke states the differential in the same breath: Belladonna has the fever, Mag phos has not.

Stomach and abdomen — the classic colic

This is the remedy's headquarters. Boericke: enteralgia relieved by pressure; flatulent colic forcing the patient to bend double, relieved by rubbing, warmth and pressure, with belching of gas that gives no relief; a bloated, full sensation with the need to loosen the clothing and walk about.

Clarke adds the geography and the restlessness: colic radiating from the navel, better for bending double or for pressure with the hand, often with a watery diarrhoea; abdominal pain that drives the patient to walk about hurriedly saying he must have relief; and the negative posture — "cannot bear to lie on back stretched out, must lie bent over." Schüssler noted amelioration from hot drinks. Allen's stomach keynote is the cramp "with clean tongue, as if a hand was drawn tightly around body" — a genuinely useful negative in a colic case, which Kent repeats. Hiccough belongs here too: Kent calls Mag phos "a wonderful remedy for spasmodic hiccoughing", admitting he has sometimes given it on that symptom alone.

Female organs

Boericke: menstrual colic, membranous dysmenorrhoea, menses too early, dark and stringy, ovarian neuralgia, vaginismus. Allen sharpens the timing into a keynote: pains worse before the flow, better once the flow begins, darting and lightning-like, worse on the right side, better from heat and bending double. Nash is emphatic: in painful menstruation "Magnesia phos. is quicker in its action than Pulsatilla, Caulophyllum, Cimicifuga, or any other remedy that I know", reserving Cimicifuga for the rheumatic subject and giving Mag phos the purely neuralgic case.

Cramps of occupation, limb and viscus

Kent is the best of the classical authors on prolonged exertion: long use of the hands and fingers in writing "gives a fair sample of writer's cramp"; pianists break down with stiffness of the fingers after years of daily practice; a labourer's hand cramps and loses hold of the implement. Boericke lists writers' and players' cramp alongside cramps in the calves, sciatica with very tender feet, twitchings, chorea and finger-tips stiff and numb; Allen adds cramps of the extremities during pregnancy. Clarke records a sciatica cured on Mag. p. 3x — pain down the right sciatic nerve after influenza, shifting, better for rest, worse at night, the parts tender and numb.

The same spasmodic action reaches the chest and the bladder: asthmatic oppression, dry tickling cough, whooping-cough and intercostal neuralgia (Boericke); spasm of the glottis, lockjaw and spasmodic retention of urine (Schüssler, in Clarke); and vesical neuralgia after the use of a catheter (Clarke). Several of these — laryngeal spasm, lockjaw, angina pectoris, the convulsions and meningitis of Clarke's cured cases — are emergencies that demand clinical assessment and referral before any remedy is considered.

Keynotes

Each is traceable to a named authority. Keynotes accelerate recognition; they do not replace the totality.

  1. Cramping, constricting pain — the single most characteristic sensation. (Nash; Allen)

Magnesia Phosphorica versus Colocynthis

In an acute colic both remedies produce the same photograph: a patient doubled over, pressing hard into the abdomen. Those two features decide nothing between them.

Kent supplies the working rule, and it turns on heat: Mag phos colic is "ameliorated by doubling up, like Coloc., and ameliorated by heat. The colic is not so markedly relieved by heat in Coloc., but is relieved by pressure." That is a statement about degree — Boericke lists warmth among Colocynthis's ameliorations too, so a case eased slightly by a hot bottle has not excluded it. What decides is which relief the patient reaches for first: the hot application, or the fist and the folded body.

The second separator is aetiology and temper. Colocynthis is, in Boericke's words, "especially suitable for irritable persons easily angered, and ill effects therefrom": angry when questioned, with cutting in the abdomen "especially after anger" and the modality worse from anger and indignation. Mag phos has no such trigger — its causations are cold, dentition, study and exertion, and its patient laments about the pain rather than resenting the questioner. The sensation differs too: Colocynthis grinds and clamps, "as if stones were being ground together in the abdomen", "as if clamped with iron bands" (Boericke), where Mag phos shoots and constricts in lightning-like paroxysms that shift about. Versus Chamomilla

Clarke and Allen both call Chamomilla the vegetable analogue of Mag phos, and Clarke gives the discriminator in half a line: Chamomilla is worse from heat. That reversal separates two remedies which otherwise share the colic, the teething and the intolerable pain. Boericke's Chamomilla is worse by heat, anger, open air and night, better from being carried, and its toothache is worse after a warm drink — the exact inverse of the Mag phos tooth that wants hot liquids. Nash notes that for infant colic Mag phos "ranks with Chamomilla and Colocynth"; the Chamomilla guide sets out that temper in full.

Versus Cuprum metallicum

Nash's shortlist for the sensation is three names: "Cramping pains. Cuprum, Colocynth, Magnesia phos." Cuprum is the most violent and most convulsive of the three: cramps and convulsions "beginning in fingers and toes", clenched thumbs, a face distorted and bluish with blue lips, a strong metallic taste, and complaints that begin on the left side (Boericke). Its amelioration is the opposite of Mag phos's: better during perspiration and from drinking cold water. Where Mag phos gives a right-sided neuralgic cramp in a conscious, complaining patient relieved by heat, Cuprum gives a left-sided, blue-faced, convulsive spasm relieved by cold drinks — see the Cuprum metallicum guide.

Versus Arsenicum album

Both remedies have marked relief from heat. But among all the varieties of Mag phos pain, Nash observes, "the one conspicuous for its absence is the one most characteristic of Arsenicum, viz. — burning pains." He then states the rule he worked by: if burning pains were relieved by heat, Arsenicum was almost sure to relieve; pains not burning but relieved by heat were cured by Magnesia phos. Burning plus better-from-heat means Arsenicum; cramping plus better-from-heat means Mag phos.

Versus Belladonna and Pulsatilla

Three connections tie Mag phos to Belladonna. Nash and Allen both gloss "lightning-like in coming and going" with Belladonna; in teething spasms Clarke states the discriminator plainly, "Bell. has fever, Mag. p. not", and Allen adds the hot head and skin; and Kent's congestive headache with red face and throbbing looks "almost like Bell." but yields to Mag phos when heat and hard pressure relieve. Boericke also lists Belladonna, Gelsemium and Lachesis as antidotes to Mag phos. Pulsatilla takes one line from Clarke's Relations: both have shifting, erratic pains and both treat dysmenorrhoea — but Pulsatilla is worse from heat where Mag phos is better.

The Tissue-Salt Question

Mag phos arrives in most curricula twice: once as a classical remedy, once as one of Schüssler's twelve tissue salts alongside Kali muriaticum and Calcarea phosphorica. What the classical authors made of that matters.

Clarke is the most generous. He reproduces Schüssler's account in full, observes that Allen's proving confirms it point by point, and draws the historical conclusion: "Schüssler arrived at them by a way of his own, which shows that there are other means besides provings of finding the keynote symptoms of remedies." Kent and Nash prescribe it on the proving instead. Kent restricts the method, not the theory: "its proving justifies its use in neuralgia ameliorated by heat and pressure, cramps and twitchings." Nash goes further — "I have no faith in the Schuesslerian theory in regard to it" — while insisting that the law of similars holds for the tissue remedies "regardless of theories."

The upshot: prescribe Mag phos on its proven picture and its modalities, as you would any other remedy. The tissue-salt lineage explains why it is so often met in low decimal potencies; it changes nothing about how the case is taken.

Dose, Potency and the Hot Water

Boericke closes his entry with: "First to twelfth potency. Sometimes the highest potencies are preferable. Acts especially well, given in hot water." Allen records the same, and the observation fits a remedy whose gastric symptoms Clarke found relieved by drinking hot water.

The potency range in the literature is unusually wide. Clarke cured a severe chorea in a girl of six with the 6x, and his own patient with cracked lips did best on the 9x after higher potencies were tried; Ord's sciatica answered to 3x in five-grain doses. At the other end, Nash prescribed the 55m habitually in neuralgic dysmenorrhoea. No single rule emerges; our guide to potency selection sets out the reasoning. Mag phos rewards a small, sharply chosen analysis. Take the modalities first, because they are the remedy:

  1. Generals: better from heat and better from hard pressure — two rubrics that together cut the field to a handful.
  2. Better from bending double — the rubric that puts Mag phos and Colocynthis side by side and forces the differential above.
  3. The sensation: cramping, and the shifting or wandering character of the pain.
  4. Worse from cold bathing or cold washing — a small, selective rubric that is often the confirming one.
  5. A right-sided general, plus a concomitant that fixes the region: spasms during dentition without fever; menses, pains better when the flow begins; cramp in the fingers from writing or playing.

Why is Magnesia phosphorica so often given in hot water?

Because the authorities observed that it works better that way. Boericke ends his entry with the plain instruction that Mag phos "acts especially well, given in hot water", and Allen records the same: it "sometimes acts best when given in hot water". The observation fits the remedy's own modality — a patient whose every pain is relieved by external heat and by hot drinks, and whose gastric symptoms Clarke notes are better for drinking hot water. It is a dispensing detail, not a dosing recommendation; potency, repetition and the decision to prescribe belong to a qualified practitioner.

Which remedies compete with Mag phos in menstrual cramps?

Nash puts Mag phos ahead of the field for the purely neuralgic dysmenorrhoea: quicker in action, he writes, than Pulsatilla, Caulophyllum or Cimicifuga, and he reserves Cimicifuga for cases of rheumatic character or in a rheumatic subject.
