Lithium has been used by dominant medicine empirically but we should consider it more often in the gouty conditions affecting the heart, and where there are rheumatic pains at the last joints of the fingers and toes.

HEART
DULL STITCHES IN CARDIAC REGION WITH SUDDEN SHOCKS; THROBBING.
RHEUMATIC SORENESS IN CARDIAC REGION.
VIOLENT PAIN IN PRÆCORDIAL REGION < BENT OVER.
SORENESS ABOUT HEART < STOOPING, ASSOCIATED WITH PAINS IN LIMBS; FINGER JOINTS TENDER AND PAINFUL.
TREMBLING AND FLUTTERING OF HEART < MENTAL AGITATION OR BEING VEXED.
BRUIT OF AORTIC VALVES.
NECK AND BACK
Sore feeling on right side of spine < pressure, < morning on rising.
STITCHING IN SACRUM < WHEN STANDING.
SUPERIOR EXTREMITIES
Pain near point of right pectoralis major on margin of shoulder.
Stitch burning in ball of left thumb.
Sensitive pains in all fingers, more pronounced in second and third, deep as if in bones; extending from hands to fingertips, only during repose; < on pressure and motion.
Left middle finger painful.
INFERIOR EXTREMITIES
Pain in right hip going to left.
Burning pain in small spot on right hip, then thigh then little toe.
RHEUMATIC PAINS IN LOWER LIMBS.
Pains in and above knees, especially going upstairs.
PAINFULNESS OF FEET, ANKLES, METATARSAL AND TOES, < AT BORDER OF FOOT AND SOLES IN GOUTY PATIENTS.
Ankle joints painful when walking, first right then left.
Rheumatic pains in right foot on waking, > on rising.
EXTREMITIES IN GENERAL
SWELLING, TENDERNESS, SOME REDNESS OF LAST JOINT OF FINGERS WITH GENERAL PUFFINESS OF BODY AND LIMBS.
ARTHRITIC PAINS IN LIMBS.
GOUTY AFFECTIONS WHEN HEART IS AFFECTED.
OBESITY WITH GOUT.
